Rhinoplasty is both a science and an art. Because the nose is the face’s most central feature, even subtle changes can improve facial balance and harmony. The goal is not to create a “perfect” nose, but to achieve a natural-looking result that complements the patient’s other facial features.
Andres Sárraga, MD, FACS, is a board-certified plastic surgeon who carefully considers both appearance and nasal function. During every rhinoplasty procedure, he evaluates the external shape of the nose, its internal anatomy, and the nasal airway. His goal is to create an attractive, balanced nose while maintaining, or improving, comfortable breathing. Whenever appropriate, Dr. Sárraga uses a Scarless Rhinoplasty technique that avoids an incision across the outside of the nose.
What Is Scarless Rhinoplasty?
Scarless Rhinoplasty, also called closed rhinoplasty, is performed through incisions hidden inside the nostrils. Unlike open rhinoplasty, this approach does not require an incision across the columella, which is the strip of skin between the nostrils.
Through these internal incisions, Dr. Sárraga can reshape the nasal cartilage and bones. Cartilage may be removed, repositioned, or added to areas such as the nasal tip, bridge, or base. When grafting is necessary, cartilage can often be obtained from the nasal septum or another area of the nose being reduced.
Small incisions may occasionally be placed within the natural creases beside the nostrils to reduce flaring or improve nostril proportion. These incisions are carefully positioned to remain as inconspicuous as possible.

Candidates may be concerned about a prominent bridge, drooping or wide nasal tip, flared nostrils, asymmetry, or a nose that appears out of proportion with the face. Rhinoplasty may also address structural problems that interfere with breathing.
Ideal candidates are generally healthy, have realistic expectations, and understand that every procedure must be customized. The Rhinoplasty Consultation
During your consultation, Dr. Sárraga will listen to your goals and concerns before examining both the internal and external structures of your nose. He will also evaluate your facial proportions and nasal airway. After the examination, he will explain his recommendations, discuss whether you are a good surgical candidate, and review the changes that may be realistically achieved. You will have an opportunity to ask questions about the procedure, anesthesia, recovery, risks, and expected results.
The Scarless Rhinoplasty Procedure
Scarless Rhinoplasty is generally performed while the patient is asleep under general anesthesia, particularly when nasal airway correction is required. Surgery may take place in the practice’s accredited surgical facility or at the adjacent Aventura Hospital and Medical Center. Most rhinoplasty procedures are outpatient surgeries. Patients may also have the option of spending the first night in a certified recovery facility with experienced nursing assistance.
At the end of surgery, Dr. Sárraga typically applies an external splint molded to the nose’s new shape. A soft internal stent or packing may also be placed to support healing and keep the airway open. Some stents contain small tubes that permit limited airflow. A gauze drip pad may be positioned beneath the nostrils to collect minor drainage during the first day or two.
Rhinoplasty Recovery
Dr. Sárraga generally examines patients the day after surgery. The external splint is usually removed after approximately one week, while internal stents are typically removed between four and ten days after surgery, depending on the extent of the procedure. Swelling, bruising, congestion, and discomfort are expected during the early recovery period. Medication may be prescribed to manage discomfort and other symptoms. Many patients feel comfortable returning to work and social activities after approximately ten days, although individual recovery times vary.
Scarless Rhinoplasty Results
Initial swelling usually decreases within several weeks, but the nose will continue to refine gradually. Complete healing and the final result may take several months to one year. Rhinoplasty results are designed to be long-lasting, and a healthy lifestyle and appropriate sun protection can help support the outcome.
